Question list

Take the questions with you.

You do not need to remember everything during a visit. Put the answers on paper, in words that make sense to you.

Who will decide on the treatment?

What is the treating person’s full name and job? Which board record can I check for the state where I will be?

What are we trying to address?

What do you think explains my concern? What else could we discuss before choosing a medicine?

Why this exact medicine?

What is the medicine’s full name? Is it a tablet, a shot or something else? What do we hope it will help with? How will we check whether it helps?

What do tests add?

What would each test change? Who arranges it, who pays for it and who explains the result?

Who makes and sends it?

Which pharmacy is involved? Can I have its name before I pay and check its record with the state board?

What might need help?

What problems can happen with this product? What needs a quick reply, and what needs emergency help?

What is the full bill?

Which services does the visit fee or membership cover? What will the medicine, tests and later care add?

What happens after this visit?

When do we speak again? What is the contact route to my treating person if a new concern comes up? What happens if I leave the service?
